Hi! I am a board certified DNP-PMHNP-BC. I started my career in traditional health practices as a Fulbright Scholar in Sochi, Russia. I presented my research at the US Embassy in Moscow. Following the completion of my grant I attended New York University (NYU). From there I completed a residency at M.D. Anderson Hospital in Houston, Texas. Following this I entered and completed my doctorate at USD University of San Diego, California with a focus on digital telehealth treatment for psychiatric care. I have a passion for working with the veteran population and those seeking care outside the box. I recently completed my facilitation for psychadelic assisted therapy and have an additional practice in Denver, Colorado. I enjoy yoga, swimming and all the water sports!
Mrs. Evon Czok is a dual-certified Psychiatric and Family Practice Nurse Practitioner with over 30 years of nursing experience. A top graduate of her class (Mount Carmel College of Nursing, 2019, Ursuline College, 2025). She is deeply committed to partnering with clients on their journey toward wholeness. Her clinical focus includes anxiety, depression, ADHD, mood concerns, binge-eating behaviors, stress, emotional health, and supporting wellness and sustainable change through life transitions.

A little background: I attended Loma Linda University School Nursing for my Bachelor of Science and RN degrees, Cal State Long Beach for my Master of Science and Nurse Practitioner degrees, and most recently UCLA for my Doctor of Nursing practice degree.
The boring details: I offer both telehealth and in person psychiatric services telehealth appointments are available to patients anywhere in California with in-person visits offered in San Bernardino and at my Brentwood satellite office by appointment.

My goal is to create a space where you feel heard, respected, and actively involved in developing a treatment plan that fits your unique needs and goals. I have over a decade of experience as a psychiatric nurse practitioner and previously spent nearly 10 years as a neonatal intensive care (NICU) nurse. These experiences have shaped my whole-personal approach to care and deep appreciation for clinical connection between physical health, emotional well-being, and family dynamics.
I offer both medication management and supportive therapy taking time to educate patients about their diagnosis and treatment options so they can make informed decisions about their own care.
